The registration form for the inaugural AfHEA conference can be downloaded (below). The form is designed to find out if you are planning to attend the inaugural conference of AfHEA in March 2009 and assist us in planning the conference.

PLEASE CAREFULLY READ THE FUNDING AND PAYMENT INFORMATION FOR THE AfHEA INAUGURAL CONFERENCE IN SECTION 3 BEFORE COMPLETING THE FORM.

The costs of attending the conference (for those who will register by 31st January 2009) are as follows:

Cost per participant including accommodation but excluding air travel: US$2,200.
Cost per participant for those arranging their own travel and accommodation: US$1,400 (includes all conference-provided meals, other meeting costs, administrative costs and conference fee).

The above costs include the conference fee of $500 per participant.

Please check carefully that you have entered the account number and codes exactly as above to avoid any delays in your payment and any associated problems regarding your attendance.

We regret that we do not have online payment facilities and automatic payment receipts system tied to our account set up as yet, and so we strongly advise that when paying into the AfHEA account, you provide a reference name or other identifying information, and in addition, please send a mail to afhea08@gmail.com to notify the conference management of your payment including the reference information provided. A receipt will then be sent to you on verification of the payment. You will need to produce this receipt on arrival at the conference registration desk.

Bank Draft made payable to: African Health Economics and Policy Association. This can be paid into our account, or could be paid on site at the opening of the conference if permission to pay on site has been obtained in advance from the conference management. Please do not travel to the conference without having paid in advance, or received permission in advance to pay on site.

Paypal: Those who wish to pay online or by credit or debit cards may choose to do so via PayPal. You must have your own Paypal account to use this method, but this is extremely easy to do (see www.paypal.com). The email address to use for payment in this case is afhea08@gmail.com Note that in this case, you need to add 3% to the total amount paid for transaction fees.

We do not encourage participants to travel with cash intended for paying for their conference costs. In very exceptional cases, where a participant cannot use any of the above methods, the conference management may accept reasonable alternatives that would not impose additional transaction costs on AfHEA. It is important to make such a case to the conference management as early as possible. But please do NOT travel to the conference without having first obtained approval from the conference management (via afhea08@gmail.com) that your alternative payment method will be accepted.
